By completing the form you will receive a monthly newsletter, by email, highlighting some of the ways you can get involved and have your say in shaping health and care services in Leeds. 

You will get the option below to tell us what you’re interested in. The more things you select, the more you’ll hear from us. If you are interested in the work of a particular organisation, you can choose to hear from them directly too (they'll then get in touch separately).

By signing up to our network, you are agreeing to share your contact details with staff members of the Leeds Health and Care Partnership who will only use your details to contact you for the areas of interest and organisations you have selected in this form. If you select 'NHS West Yorkshire Health and Care Partnership' your information will be available to the staff members there so they can get in touch with you. They will contact you about the 'West Yorkshire Voice' network.

The Leeds Health and Care Partnership are health and care organisations from across Leeds that are working together to improve the health of the people in Leeds. This includes NHS organisations, Leeds City Council, and voluntary sector organisations.
